Findings

This protein-truncating variant was found in a control subject, in a Caucasian cohort from Britain and North America, consisting of 332 Alzheimer’s cases and 676 controls (Sassi et al., 2016). Although further information was not provided about the individual carrier of this variant, the mean age of inclusion for the control group in this study was approximately 78 years, with a range of 60 to 102 years.

The variant subsequently was identified in a dataset from the Centre National de Référence - Malades Alzheimer Jeunes (CNR-MAJ), the French national reference center for young Alzheimer patients (Schramm et al., 2022). The proband was diagnosed with probable AD with symptom onset at 59 years; their APOE genotype is E3/E4. Multiple family members were also found to carry the variant: a sibling of the proband, diagnosed with AD at age 61 (APOE E3/E4), one of their offspring, cognitively intact at age 42 (APOE E2/E3), and, notably, a parent of the proband (APOE E3/E3), cognitively intact at 95 years of age. An unaffected sibling of the proband, cognitively intact at age 68, does not carry the variant. Genotype information was not available from two other siblings of the proband, unaffected at ages 59 and 69, respectively.

In a study that included 15,808 Alzheimer’s cases and 16,097 control subjects from multiple European and American cohorts, including CNR-MAJ, this allele was observed twice among the AD cases (Holstege et al., 2022).

Functional Consequences

This variant was predicted to be deleterious by SIFT, probably damaging by PolyPhen-2, and disease-causing by Mutation Taster (Sassi et al., 2016).
